In 2000 plans were approved by the Dartford and Gravesend NHS Trust for a modern community hospital with 80 beds to be built on the site of the Gravesend and North Kent Hospital.  It was intended to be completed by 2003 but, in the event, the old Hospital did not close until 2004.

Following demolition of the old buildings, a foundation stone for the new Hospital was laid on 15th September 2005.

Built under a £8m P.F.I. scheme, the Gravesham Place Integrated Care Centre opened in June 2006 with 104 beds.  The complex also contains the Gravesham Community Hospital and a Day Centre.  It is run by the Kent Community Health NHS Foundation Trust.  The Medway NHS Foundation Trust and the Dartford and Gravesend NHS Trust provide some services.

The Out-Patients Department of the Hospital provides a wide range of services, with clinics for audiology and sexual health.  Departments include X-ray, physiotherapy and phlebotomy (blood testing).  There is also a Minor Injuries Unit.

The Sapphire Unit specializes in rehabilitation and contains 15 beds for patients with neurological disorders and 6 for those with non-neurological conditions.

The Gravesham Place Integrated Care Centre is run by Kent County Council.  It consists of four units (Emerald, Topaz, Opal 1 and Opal 2), which provide different levels of healthcare - long or short stay, and respite care - for patients aged 55 years or over.  The Care Centre has 80 single en-suite rooms.

Present status (November 2009)

The Hospital remains operational, providing various out-patient services and therapies.

Update:  January 2016

A £126m contract to run the services at the Hospital and also at the Livingstone Community Hospital was awarded to Virgin Care.
